Born June 19, 1997 in Boston, Jake died suddenly on July 1, 2023 while in the care of Aurora Recovery, 5035 Amestoy Avenue, Encino, California, following a lengthy battle against addiction. He is survived by his mother, Anna Makos of Hampton, NH and was predeceased by his father Kenneth Stevens of Newburyport. He is also survived by his two daughters Indigo and Storm of Seabrook, NH. Jake was predeceased by his maternal grandparents Nicholas and Janice Makos as well as paternal grandparents Charles "Shiner" and Barbara Stevens, all of Newburyport. He is additionally survived by several aunts, uncles, cousins and friends. Those wishing to honor him are asked by his family to make a donation in Jake's name to a charity of their choice.
Jake will be loved and remembered forever in the hearts of all who knew him.
